  • Traditional Preferred Provider Organization Plan (PPO).
  • Sherpa pays at least 50% of EE coverage.
  • Eligible after completion of 90 days of continuous employment.

  • Periodic oral exams, cleaning, x-rays.
  • Includes various coverage for basic and major services and orthodontia.

Vision

  • Annual vision exam 1 per 12 months.

  • Employee Contributions always 100% vested.
  • Employer discretionary contributions based on a six-year graded vesting schedule.
  • Employer match set each year and historically has been 50% match of first 6% but is subject to change.
